Assessment

If you would like to avail of my service, I will complete a detailed OT assessment in your child’s home and school/preschool or early years setting.   This will include the use of standardised assessments, observations and informal play. I will work with supporting adults to build a picture of what is going well and what challenges are experienced. 

 

 

The assessment process involves

  • Initial detailed video call or phone call with parent/caregiver (up to one hour)               
  • Assessment day in school/preschool/early years setting followed by a home visit
  • Assessment scoring, formulation and bespoke report with recommendations
  • Follow up contact with family and all professionals involved to ensure everyone is confident in following through with recommendations

Tax Relief Information

Tax Relief for Medical Expenses with Revenue Commissioners: You may be able to claim tax relief on 20% of the expenses incurred for Occupational Therapy services by submitting a Form MED1 which can be downloaded here or through the Revenue Online Service (ROS).

Occupational Therapy services are covered by most health insurers with many now offering very generous OT benefits. For example, VHI PMI 25 11 covers 75% of OT fees (unlimited appointments). Many insurers offer policies for children-only (i.e. adults in the family do not need to have the same policy). You should contact your insurance provider/check your policy document to clarify what your plan covers. In addition, you may be able to claim tax relief on certain unclaimed medical expenses by submitting a Form MED1 which can be downloaded from www.revenue.ie
